Documents Required to Process a Family Exclusion Request

To successfully process a Family Exclusion request, the following documents are typically required:

1. Valid IDs: ID copies for both the individual requesting the exclusion and the person being excluded. 

2. Proof of Relationship: Such as a Birth Certificate, Marriage Certificate, or other relevant documentation. 

3. Completed Responsible Gaming Form 3 (RG Form 3): You can access the form here: RG Form 3

For foreign applicants, the documents must be issued by the applicant’s home government and must be authenticated by the Department of Foreign Affairs (DFA) of the Philippines.
