Mga Documents na Kailangan Para Mag-Process ng Family Exclusion Request
Para ma-process ng maayos ang Family Exclusion request, usually kailangan ang mga sumusunod na documents:
1. Valid IDs: Kopya ng ID ng taong nagre-request ng exclusion at ng taong ie-exclude.
2. Proof of Relationship: Pwedeng Birth Certificate, Marriage Certificate, o iba pang relevant na dokumento na magpapatunay ng relasyon.
3. Completed Responsible Gaming Form 3 (RG Form 3): Pwede mong i-access ang form dito: RG Form 3

Para sa mga foreign applicants, ang documents dapat galing sa home government ng applicant at kailangang authenticated ng Department of Foreign Affairs (DFA) ng Philippines.
